With Plus plans – you have flexible options!

  1. You may use this search tool and choose a participating provider* in the Galaxy Health Network. Galaxy providers offer services to our members at a reduced cost. Present your Insurance Benefits identification card at the time of visit and the provider will submit the claim. You will receive a bill for the reduced cost services less any reimbursement due under your plan**, or

  2. You may want to learn about additional savings opportunities by calling Customer Care at 1-800-337-1421 between the hours of 8:30 a.m. and 5:30 p.m. Eastern Standard Time, or

  3. You may see any provider, anywhere, pay for the office visit and file a claim for reimbursement.
